Anyone can make a referral to home care services. If you need home care services or know someone that does, you can contact us or your physician. If a person qualifies for our services, we will contact their physician for orders.
We provide Medical/Skilled and Non-Medical/Unskilled Services.
Coverage for Services Sources for payment can include Medicare, private insurance, and private pay. In addition, some services are covered under public and community programs. All claims will be filed for the client through our office. Specific Medicare Guidelines Services for home care are covered under Medicare based on specific guidelines. First, you must have a skilled need such as nursing services or therapy services. Second, you must have a physician’s order for the services. Third, you must meet the homebound criteria. In order to meet homebound criteria it must be difficult and taxing for you to leave the home and require assistance.

  • Medical Services (Skilled)
    • Following an initial assessment, a plan of care is decided with the client, their families, and their physicians.
    • Care is provided under a physician’s orders.
    • Services include skilled nursing and therapy. In addition with a skilled service an aide for personal care, if needed, can also be provided.
    • Based on the person’s needs, a frequency and duration of visits will be established. We continue to update the plan of care as needed per any change and with communication and approval per the ordering physician, client and their family
  • Non-Medical Services (Unskilled supportive care)
    • Homemaking and aide services for personal care are provided under the supervision of a nurse.
    • A physician’s order is not needed for this service.
    • The frequency of visits is based on the services and needs of the client and the family.
